What It Is, What It Measures, What It Means

Hemoglobin Electrophoresis is performed when a blood disorder associated with the normal forms of hemoglobin is suspected. If average blood glucose over a three month period does not coincide with the expected value of HbA1c, this test might reveal various types of anemia, or a blood disorder characterized by different types of hemoglobin A. The most common forms of the hemoglobin molecule are HbA, HbA2, HbF, HbS, HbC, Hgb H, and Hgb M. HbA is subdivided into A1, and A2. In normal adults, only HbA1 and HbA2 are present at significant levels. Small amounts of HbF (which is the major Hb present in the fetus) may be present, but they are of no consequence unless levels are more than 2% of the total. Hb A1 is further subdivided into HbA1a, HbA1b, and HbA1c. HbA1c is particularly useful in diabetes management because it is the hemoglobin that has been damaged by a glucose molecule stuck to it, and it is directly related to the average blood glucose. HbA1c allows us to estimate average blood glucose levels for the past 2 to 3 months. Normal values of A1c are about 4.5% to 5.0%. 5.5% is already a little high, and above 6.0% there are progressively worse side effects. See: The Bernstein HbA1c to Average Blood Glucose Conversion Table. HbS is an abnormal form of hemoglobin associated with sickle cell anemia. HbS forms clumps in the presence of low oxygen, causing the red blood cells to take on a crescent or sickle shape. These misformed cells then break down, and can block small blood vessels. HbC is an abnormal form of hemoglobin associated with hemolytic anemia. The symptoms are much milder than they are in sickle cell anemia. Other, less common, abnormal Hb molecules cause anemias of various degrees of severity. Normal Values In adults, these hemoglobin molecules make up the total hemoglobin, as follows: Hgb A1: 95% to 98% Hgb A2: 2% to 3% Hgb F: 0.8% to 2% Hgb S: 0% Hgb C: 0% In infants and children, these hemoglobin molecules make up the total hemoglobin, as follows: Hgb F (newborn): 50% to 80% Hgb F (6 months): 8% Hgb F (over 6 months): 1% to 2% What abnormal results mean The presence of significant levels of abnormal hemoglobins may indicate: Hemoglobin C disease Rare hemoglobinopathy Sickle cell anemia
